Mrs. Mayra C. Grimaldo-Cagganello, Coordinator of Health Education & Chronic Disease Management Services at Charter Oak Health Center.

Mrs. Cagganello is an International Medical School Graduate and trained physician from Venezuela. She graduated with a Master of Public Health degree from Southern Connecticut State University. In addition, Mrs. Cagganello holds a national certification, namely: Certified Health Education Specialist (CHES), which has prepared her to carry out different professional roles in the field of health care, community health services and research. Ms. Cagganello has been working as a health educator and program developer and evaluator of multiple diabetes educational programs in the United States since 1993. At Charter Oak Health Center, Mrs. Cagganello helps individuals to overcome barriers to diabetes self-management by offering individual and group diabetes education, medication adherence counseling and case management services. She is the leader of the Diabetes Care Team. As a public health care practitioner, she collaborates with other teams in the development and evaluation of different interventions to enhance the care provided to patients with chronic conditions, such as: diabetes, hypertension and asthma and depression.
