Outreach
Outreach provides effective strategies
and plans that will provide maximum
exposure of COHC services to the
community. We also coordinate and
conduct structured visits to homes,
businesses, schools, organizations and
to community partners to expand our
patient base. We coordinate Agency
participation in community events,
health fairs, food share and related
events. We find clients for COHC
providers who cannot reach their client
since these individuals do not have
telephones or they do not respond to our
letters.
We
are responsible to facilitate a broad
awareness of our health center programs
and services and to promote access to
care and collaboration to all.
We
meet individuals where they are and to
provide them with the necessary
information that will attract them to be
clients of COHC. To follow up
individuals with mail-outs, telephone
calls and face to face calls. To
maintain a positive public relations
presence while maintaining excellence in
the delivering of our health care
services. To educate and assist
individuals in accessing our services
through a communication that can be
easily understood. To stay aware of
those issues and concerns that that
would cause individuals to leave COHC. |
